Entry forms are available at and can be returned to the Guidance Office at Lunchtime on Mondays, Wednesdays, and Fridays now through May 23. Entry forms are also available at www.cchsboosters.com or from Booster Club members at school events.

One entry per graduating senior household is permitted. There is a suggested donation of $20 per entry to support the CCHS Booster Club efforts to support our athletic teams and extracurricular programs.
